Contest Terms & Tax Responsibility

Participation in the #CaptureHalloweenSpirit Costume Contest is entirely voluntary and open to CaptureRx’s community of Covered Entities. Through the submission of participant’s photos to the #CaptureHalloweenSpirit Costume Contest, each participant affirms their rights to use the photo and grants to CaptureRx the right to use the participants’ likeness, photo, and/or video in publications, news releases, online or in other communications.

Prizes awarded in the #CaptureHalloweenSpirit Costume Contest are cash or cash equivalents which are subject to federal taxation and may be subject to state or local taxation. Winning participants are solely responsible for any applicable taxes associated with the prizes.

Additionally:

  • Only submit photos of you; do not submit photos of other persons.
  • Through the submission of your photos to the #CaptureHalloweenSpirit Costume Contest, you affirm your rights to use the photo and grant to CaptureRx the right to use your likeness, photo, and/or video in publications, news releases, online or in other communications.
  • Submit your name, address, email, and mobile telephone number to be contacted for prizes.
  • Persons who do not complete the submission paperwork will be rejected and their photos ineligible for prizes.

The closing date for entries will be released in the next few days. Please stay updated by checking our Twitter and LinkedIn pages. To submit:

  • Tag @CaptureRx on Twitter or add your photo in the comments of our LinkedIn post, ensuring you use #CaptureHalloweenSpirit.
  • Include contact details. If you’re uncomfortable sharing this publicly, email your details and associated picture to mitchell.sullivan@capturerx.com. Remember, to participate, you must post your photo on Twitter or LinkedIn.

Notes

If company policies prevent you from accepting cash prizes, CaptureRx will donate the winnings to your entity as a grant.
